TAP for Transition Hangout: Utilizing Approved Transition Assessment Tools to Facilitate Learning

The Guide to Creating and Using a Transition Assessment Toolbox is intended to facilitate the development of a systematic approach to transition assessment. This guide is a companion tool to the Transition Assessment Planning Form, which provides a fillable template that follows a student throughout the transition planning process. Together, these tools assist Educational Organizations, students, and families in developing quality transition-focused IEPs. This hangout will allow participants to consider and discuss various opportunities to incorporate both tools into their work and describe and share ideas for using them effectively.
